Please fill out this form only if you, the Main Applicant, meet any of the following conditions:

1. You will travel to a different address before or on May 26th.
2. The address you submitted on the Al Bait Registration Form is incorrect.
3. You rather prefer to use your expedited shipping label you can mail us at [email protected]. It is a document letter no more than 8 ounces in weight.
4. You would prefer to receive the luggage tags in Madinah rather than in the USA.

Please Note:

• Only the Main Applicant will receive the luggage tags by mail for the entire group.
  If you are not the main applicant and are traveling, you do not need to fill out this form.
• Documents and tags will only be mailed to the addresses provided through the Al Bait Guest Forms.
